Once you book your 1-hour initial appointment below, you will be emailed pre-visit paperwork to complete and sign electronically. Paperwork must be completed no later than 2 business days prior to your appointment, in order to verify insurance coverage. Appointments can be cancelled 24 hours or more before their start time at no cost. Any appointment cancelled less than 24 hours before its start time (or not attending your appointment) will result in a $30 fee. After your appointment, the credit card you provided in your electronic paperwork will be charged according to your insurance coverage.
